Coverage summary
- Annual eye exam ― Covered in full.
- Frames ― $200 allowance.
- Lens ― Single vision, lined bifocal, lined trifocal or lenticular lenses come with prescription glasses.
- Contact lenses (instead of glasses) ― A contact lenses exam and a $190 allowance toward the cost of contacts.

Seeing a vision provider
Use the guidelines below for what you should know and do when seeing a vision provider.
- Visit www.CignaEnvoy.com to find a provider.
- When scheduling your appointment, provide the name of the vision company and plan under which you are covered, and the ID number.
- If you require prescription eyewear, your provider may also help you pick out eyeglasses or fit you with contact lenses during your appointment. Your provider will apply any discounts and you'll pay for your eyewear at the time of service.
